The PDF file is formatted for you to be able to type your name and date in nicely. This needs to be done on a desktop!
IF YOU HAVE A PC and not a MAC, download the PC version and add a text box if your program has that ability. If not, simply print and handwrite your name and date.
STEPS FOR MAC COMPUTERS:
1.) Download the PDF file to your computer (this cannot be done on mobile phones).
2.) Open the PDF.
3.) Click on the word, “Name” and a text box will appear for you to delete “name” and type yours in! The same for where it says, “Date”.
I designed this to fit on a normal sized printer paper of 8.5 x 11 inches (21.59 x 27.94 cm).

Hi! First thank you for this download!! I love it! Second, how do I print it with my name and date, instead of Name and todays date that is written on the download. Thanks!!
Hey Stacy! I’m glad you love it! Once you download the PDF, it will save on your computer (this needs to be done on a desktop, not on mobile phones) then you can click on the word “Name” and it will put a cursor there for you to type it right in! Same for where it has “Date” 🙂 Let me know if you need more help to get it to work!
Sorry. I still cant get it to work. I downloaded it twice. I will try again! Thank you. It isnt letting me click on name. Maybe its my program!?
Aw it could definitely be the program. I would check to see if the PDF program that opens it for you is updated.I open mine on a Mac and it uses the “Preview” program.
I dont have Mac, i have Windows. Might be the issue. Its ok. Thank you
Hey Stacy, I just added a PC version to download that doesn’t have the word, “Name” or “Date” so you can print it and handwrite it in! My Windows friends said you would need to add the text box yourself if you have the version of the PDF program with that option. On a MAC it has that automatically. Let me know if you have any other issues with it working!
I have tried everything and cant remove “name”.
